Nobody picks a surgeon the way they pick a chiropractor
A patient looking for an adjustment searches, looks at the three businesses on the map, and picks one within a few miles. The whole decision can happen in under two minutes, and proximity decides most of it.
A rhinoplasty patient behaves nothing like that. The research runs for weeks or months. Board certification gets checked, often on a separate site. Before-and-after galleries get compared across four or five practices. Somebody who will fly to another state for the right surgeon is not going to pick the nearest one because Google put it first.
Which means the advice a general marketing agency gives you is probably wrong for your practice. Optimizing for the map pack, the three businesses Google shows on the map, is not useless for a surgeon. It is just not where the leverage sits.

Most before-and-after galleries are image sliders with no text around them, which means the most persuasive asset in the practice is invisible to search. Procedure name, technique, and a short description around each set changes that.

The closest result we have, and what it is
We have not run a plastic surgery practice. We have run aesthetic practices in the same markets, competing for the same patients, and that is the honest adjacent case.
A South Florida med spa. On its core treatment search, top-three coverage across a 49-point grid went from 6 points to 36. Average position across the grid went from 18.3 to 2.7, where lower is better, and it still held that coverage at the most recent scan on 12 September 2026.
That is a med spa and not a surgical practice, and we are labeling it that way on purpose. What transfers is the method and the market. What does not transfer is the patient, who researches longer and travels further for a surgeon than for an injectable.

The audits say the same thing from the other direction
The anonymous medical-spa sample had 358 reviews at 4.9 stars, yet appeared in the top three on only five of 245 service-and-location checks. It did not appear in the top 20 on 68% of them.
This is a prospect finding, not a surgical-practice result. It shows why a good reputation and broad local visibility should be measured separately.
